That is a tricky question but authors usually make a plan on what they visualize for the future of their book article etc. Or they go along as the story progresses
Conflict is simply an issue between two characters, a character vs. nature, character vs. himself, etc. An author creates the conflict by creating an issue/problem for the characters to solve.
